In the later expansions, the drop rate of this mount was fixed at 1%, but on WotLK it's a lot more reliable. How to get Mimiron's Head in WotLK Classic

Mimiron's Head is a pretty unique mount, and it has a 100% chance to drop from Yogg-Saron in 25-player Ulduar. The catch is that you have to do it without any Keepers helping you. What does that mean?

Each of the four Keepers will boost your damage output by 10%, up to 40% in total. You will also have an option to evade death with Hodir's Protective Gaze, and restore your Sanity with Sanity Wells. Titanic Storm will kill Immortal Guardians of Yogg-Saron, and Destabilization Matrix will slow down his attacks. They're essential for having the fight not be incredibly difficult.

Having to fight Yogg-Saron without any of those bonuses means that you will have to have a well-coordinated team to tank Immortal Guardians, not lose too much Sanity, control his tentacles, and keep everyone alive. It's a very challenging encounter that requires experienced teammates. That's why not everyone has this mount.
