3. Keep Talking to Complete Nut
Do not leave after the first line of dialogue. You need to interact with Complete Nut several times.
He starts with a simple “Moo” and continues insisting that he is a cow. Keep speaking to him until the dialogue changes and he realizes that you have blown his cover.
This interaction is the trigger for the actual encounter. If nothing has attacked you yet, talk to him again.
4. Kill the Cows Coming Through the Portals
After Complete Nut’s cover is broken, portals open around the arena and hostile cows begin spawning.
Kill the cows and clear the encounter. There is no separate puzzle to solve during this part. The important step is finishing all of the enemies that appear after the dialogue trigger.
5. Talk to Complete Nut Again
Once the cows are dead, return to Complete Nut and interact with him one more time.
He eventually responds with: “Well, thanks a lot! Now I gotta change.” The Waking Nightmare is then cleansed and the completion loot appears.

Check the rewards for the Head of the Bovine. On PC, right-click the item in your inventory to unlock the cosmetic. It can then be used as a head transmog.

If you reach Complete Nut and nothing seems to happen, the first thing to check is whether you have exhausted his dialogue. Walking away after one or two interactions means the cow portals never get their trigger.
Why Complete Nut Is in the Diablo Hellfire Nightmare
Complete Nut is an old Diablo: Hellfire character, so his appearance here is deliberate. Hellfire had a hidden cow-themed quest in which Complete Nut could replace Lester the Farmer. Players could talk to the man in the cow suit and eventually complete his strange request for a bovine-themed reward.
The old quest rewarded the Bovine Plate. Season 15 brings the joke back through a Waking Nightmare, keeps Complete Nut as the NPC, surrounds him with cows again, and attaches another Bovine reward to the encounter.
