| Faceless Mask of the Pained | 2 Madness affixes instead of 1 | Complete Old Town in Stormwind with 1 mask active |
| Faceless Mask of Dark Imagination | While below 50% Sanity, a Horrific Figment will spawn and attack you | Complete Mage Quarter in Stormwind with 1 mask active |
| Faceless Mask of Vengeance | Killing enemies gives remaining enemies stackable damage buffs | Find the Faceless Mask of Vengeance in Stormwind Canals with 1 mask active |
| Faceless Mask of the Nemesis | Every 25 kills spawns an Elite nemesis mob | Defeat Hogger in Stormwind and Gamon in Orgrimmar, then fuse their Nemesis Shards |
| Faceless Mask of Multitudes | More enemies will spawn | Complete all five objectives in one Horrific Vision run with 3 masks active |
