Ultimate rewards are mostly cosmetic, but they stay valuable because the fights remain synced. A clear gives the encounter achievement, a title, and a totem that can be exchanged for one weapon from that same Ultimate.

FFXIV Ultimate Lockouts and Totem Farming
Current Ultimate raids have a weekly reward restriction. Dancing Mad Ultimate is the current Ultimate, so its main reward is limited until Square Enix removes the restriction in a later patch.
Legacy Ultimates have no weekly reward lockout. UCoB, UWU, TEA, DSR, TOP, and FRU can be cleared more than once per week for extra totems. This is useful if you want several weapons from the same Ultimate set.
Totems are tied to their own Ultimate. An Omega Totem is used for TOP weapons, an Oracle Totem is used for FRU weapons, and a Dreadwyrm Totem is used for UCoB weapons. One Ultimate's totem cannot be exchanged for another Ultimate's weapon set.
FFXIV Ultimate Unlock Requirements
Ultimate raids require a pre-made full party of eight players. They are entered through Raid Finder after the selected encounter has been unlocked.
Each Ultimate has its own Savage clear requirement. UCoB requires Deltascape V4.0 Savage, UWU requires Sigmascape V4.0 Savage, TEA requires Eden's Gate: Sepulture Savage, DSR requires Asphodelos: The Fourth Circle Savage, TOP requires Abyssos: The Eighth Circle Savage, FRU requires AAC Light-heavyweight M4 Savage, and Dancing Mad Ultimate requires AAC Heavyweight M4 Savage.
Ultimate raids do not allow Unrestricted Party. Limited jobs are also not allowed. The duty stays synced, so level and gear advantages cannot bypass the core mechanics.
Piloted vs Self-Play Ultimate Boost

In a piloted Ultimate boost, an assigned raider plays your character while a fixed static clears the encounter. This lets the group control position, mitigation timing, job execution, and phase order.

Ultimate Difficulty by Encounter
UWU and UCoB are the most accessible entry points for a first Ultimate reward. They are still synced duties, but older strategies and current player familiarity make them easier to plan than newer Ultimates.
TEA is a middle option for many players. It is older than DSR, TOP, FRU, and Dancing Mad Ultimate, but it still requires clean phase knowledge and steady execution.
DSR and TOP are harder modern Ultimates with strict body checks, long pulls, and low tolerance for deaths. Pick them when their title and weapon set are the main goal.
FRU is a Dawntrail Ultimate with Eden themed rewards and no weekly reward lockout. Dancing Mad Ultimate is the newest current Ultimate and has the weekly reward restriction.
